PARENT/GUARDIAN INFORMATION
Almost there... We will need a copy of the Baptism and First Communion records. Instructions will follow after registration.
Remember, God doesn't call the equipped, He equips the called!
We will need several volunteers for both class small group leaders and chaperones (moms and dads!) at the retreat. All volunteers will need to be Safe Environment certified and will be provided training before the beginning of the Confirmation preparation program.
Special Needs Information: Please identify any specific needs or challenges your child may have that require special attention by the program director or volunteer group leader. Please list any identified learning disability, classroom difficulties, medical conditions, allergies, etc.
Photography Release: From time to time, pictures and video may be taken of events and gatherings. We would like to be able to use these photographs and videos for flyers, social media postings, parish and diocesan publications, and the ministry website. If there are concerns about pictures or videos posted on the website or social media, please contact the ministry coordinator or webmaster, and they will be promptly removed.
I/We, the parent(s)/guardian(s) of the youth listed above authorize and give full consent, without limitation or reservation, to Mary, Queen of Peace to publish any photograph or video in which the above named youth appears while participating in any program associated with Mary, Queen of Peace ministry. There will be no compensation for use of any photograph or video at the time of publication or in the future.
Group photo release: At the end of the Mass of Confirmation, Mary, Queen of Peace would like to invite a photographer to take a group photo for our archives. This group photo may be displayed in our hallway of the Church basement. Additionally, we would like to share this group photo with each family through the use of MQP e-mail or other MQP-authorized communication platform, such as Flocknote.
I/We, the parent(s)/guardian(s) of the above named youth authorize and give full consent, without limitation or reservation, to Mary, Queen of Peace to display the group photo, which will include my teen, in the church basement. Additionally, I consent to the distribution of this photograph through MQP e-mail or other MQP-authorized communication platform to each respective family participating in Confirmation this session.
PARENTAL PERMISSION: I hereby give permission for my child to participate in Mary, Queen of Peace Confirmation preparation for the 2023-24 academic year. In the event of an emergency, I give permission for my child to be transported to a hospital for emergency medical or surgical treatment.
