Parents/guardians are required to serve at The Rock Youth Ministry a minimum of three times per year. This averages out to once every three months during Fall, Winter, and Spring quarters. We will contact you to set up your dates of service.

Special Needs Information: Please identify any specific needs or challenges your child may have that require special attention by the program director or volunteer group leader. Please list any identified learning disability, classroom difficulties, medical conditions, allergies, etc.
Photography Release: From time to time, pictures and video may be taken of events and gatherings. We would like to be able to use these photographs and videos for flyers, social media postings, parish and diocesan publications, and the ministry website. If there are concerns about pictures or videos posted on the website or social media, please contact the ministry coordinator or webmaster, and they will promptly be removed.
I/We, the parent(s)/guardian(s) of this youth authorize and give full consent, without limitation or reservation, to Mary, Queen of Peace to publish any photograph or video in which the above named student(s) appears while participating in any program associated with Mary, Queen of Peace ministry. There will be no compensation for use of any photograph or video at the time of publication or in the future.
PARENTAL PERMISSION: I hereby give permission for my child/children to participate in Mary, Queen of Peace Youth Ministry for the 2023-2024 academic year. In the event of an emergency, I hearby give permission to transport my child(ren) to a hospital for emergency medical or surgical treatment.

Donate a Dinner Make a donation to pay for a weekly youth group night's dinner (average dinner cost each week $80-90)
Games, Games, Games! At The Rock we like to have FUN! Make a donation towards purchasing new recreation games. ($40)
What's That Sound? Make a donation towards buying a new portable speaker for our middle school and high school youth group so we play some tunes during our youth nights! ($200)
Retreat Refresh Make a donation towards a youth's retreat cost so they can have an opportunity to experience a retreat for the first time! ($150)
Formation Fund Make a donation towards purchasing new material for curriculum for our youth group. ($60)
Catholic Speaker Fund Make a donation towards the fund for paying for dynamic Catholic speakers to speak to our youth about the faith! This will help cover their airfare, hotel, stipend and traveling costs. ($300)
